Repair Form
The slip a customer fills in when they drop a device off — their name, a callback number that is not the phone being repaired, and the screen PIN, password or draw pattern so the tech can get in. The boxed half is yours: model, repair, what was quoted and what was paid. It is a quarter sheet, so one printed page is four blank forms; cut along the dashed lines.

Repairs
Photograph the drop-off slip and it is read into a ticket for you to check. From then on the ticket is the repair: move it along the board and the customer gets a text or an email each step. The screen lock from the slip stays sealed and is shown only when you ask.

Account Form
The paper form a walk-in fills out before you key the account into RTPOS — name, address, email and security PIN. Every security question carries its own answer line, so an answer can never land against the wrong question; the customer answers any three and leaves the rest blank. The boxed strip at the foot is yours — write the numbers created there by hand after activation. It is a half sheet, so one printed page is two blank forms; cut along the dashed line.

Photograph a finished form and upload it — several at once is fine. Each is read and then waits below for you to check; nothing is filed until you confirm it. Make sure the activated number in the “Store use only” strip is written in and legible, because that is what ties the form to its RTPOS invoice.
Review queue
Open each one, check it against the photo, then file it. Filing matches the activated number to its RTPOS activation and adds the customer to Contacts with the email and address the form supplies — the two things the activation report never carries. Anything that could not be read shows Type it in: same screen, blank fields, files exactly the same way.

What Laser Light Company would charge to rent what you already own. They list nothing under 6 W, so your ten 3 W heads are priced at the 6 W Clubmax rate, the closest thing they rent. Lasers rent in pairs, so eleven heads would bill as twelve. Shipping is free on weekly and monthly rentals booked ten business days out; daily rentals ship at $1.50 a pound each way.
